The Super Star in Community Nursing Award has been suspended indefinitely. Accepts the Chicago Area Grant Application Form. Faxed or e-mailed letters of intent and proposals will not be accepted. Full proposals are invited after review of letter of inquiry. Application form required. Applicants should submit the following:
