Procedure:
Eligible organizations include those with 501(c)(3) status, churches, public and charter schools, nonprofit hospitals, university and college programs, and other organizations recognized as exempt by the IRS but that do not have 501(c)(3) status; all eligible applicants must complete an online questionnaire to determine eligibility before being considered for grants. Application form required. Applicants should submit the following:
1. Signature and title of chief executive officer
2. Copy of IRS Determination Letter
3. Copy of most recent annual report/audited financial statement/990
4. Listing of board of directors, trustees, officers and other key people and their affiliations
5. Copy of current year’s organizational budget and/or project budget
6. Additional materials/documentation
