Organizations may be invited to submit a formal application for Formal Funding Grant Program and Brown County T.E.A.C.H.E.R. Fund. Support is limited to 1 contribution per organization during any given year. Organizations receiving support are asked to submit a final report. Application form required. Applicants should submit the following:
1. Timetable for implementation and evaluation of project
2. Population served
3. Copy of most recent annual report/audited financial statement/990
4. How project’s results will be evaluated or measured
5. Detailed description of project and amount of funding requested
6. Copy of current year’s organizational budget and/or project budget
7. Listing of additional sources and amount of support
8. Listing of board of directors, trustees, officers and other key people and their affiliations
