Applications should include a cover letter signed by the organization’s Executive Director or President. Proposals should be no longer than 5 to 6 pages. Support is limited to 1 contribution per organization during any given year. Application form required. Applicants should submit the following:

5. Brief history of organization and description of its mission
6. Geographic area to be served
7. Copy of most recent annual report/audited financial statement/990

10. Detailed description of project and amount of funding requested
