Procedure:
Guidelines must be reviewed prior to submitting a request. Proposals must be signed by your Executive Director and be a maximum of six pages. Please include the following information within your proposal:
Name, address, telephone number, fax number, e-mail address, and website address of your organization
Name, title, phone number and e-mail address of contact person
Charitable registration number
Mandate or mission of your organization
Project name
Brief (one paragraph) summary of the project proposed
Amount requested
Time frame for which funding is requested
Project purpose and the need/opportunity which prompted it
Overall goals as well as specific and measurable objectives of the project
Method of project evaluation
Past achievements of your organization which makes it qualified to undertake this project
Full project budget that clearly shows the amount requested and how it will be applied
Detailed work plan with corresponding timelines
Please attach the following items to your proposal:
Most recent financial statements (audited if possible)
List of the Board of Directors
Annual report (if available) or other printed material describing the organization that is available to the community served.
