Carolyn Foundation Coversheet required and Minnesota Common Grant Application Format preferred. Visit foundation web site for updated application guidelines, download of application and cover sheet forms, and procedures. Grant requests not to exceed 6 pages. Application form required. Applicants should submit the following:
1. How project will be sustained once grantmaker support is completed
2. Signature and title of chief executive officer
3. Principal source of support for project in the past
4. Copy of IRS Determination Letter
5. Copy of most recent annual report/audited financial statement/990
6. How project’s results will be evaluated or measured
7. Listing of board of directors, trustees, officers and other key people and their affiliations
8. Copy of current year’s organizational budget and/or project budget
9. Listing of additional sources and amount of support
