Additional information may be requested at a later date for the Champ’s Children’s Health and Wellness Grant Program and regional or national organizations receiving Bearemy’s Kennel Pals Program grants. A personal interview may be requested for the Champ’s Children’s Health and Wellness Grant Program. Generally, support is limited to 1 contribution per organization during any given two-year period for the Bearemy’s Kennel Pals Program. Organizations receiving Bearemy’s Kennel Pals Program grants may be asked to provide a final report. Application form required. Applicants should submit the following:
1. Copy of IRS Determination Letter
2. Copy of most recent annual report/audited financial statement/990
3. Listing of board of directors, trustees, officers and other key people and their affiliations
4. Copy of current year’s organizational budget and/or project budget
